The application for registration shall be accompanied by payment of non-refundable fees as follows:

• K 50 Application forms
• K 100 Printing of Certificate

In case of loss of a certificate of registration, the supplier shall be required to pay a fee equivalent to the cost of the certificate for replacement upon submission of a police report and an accompanying letter requesting for the re-insuance of the certificate.
